Just for double check, are you sure, that -5V is pin 4 from the right?
On the schematics, it is pin 3.
Same question with 12V, it is pin 6 on schems.

Paul was so kind and helped me identify this one, it is the only known sound pcb from a Zaccaria Laser Battle (Lazarian) set
